One important factor in the safety of community-based birth is the capacity for seamless transfer from one setting to the other. Countries with the best outcomes uniformly have more integrated systems for transfer from home or birth center to hospital than we do in the U.S. Furthermore, by integrating deep listening and full consent procedures, transfers can be accomplished to ensure racial and gender equity and trauma-informed care. In 2013, the Home Birth Summit gathered leading thinkers from all stakeholder groups in the U.S., including physicians, midwives, nurses, doulas, childbirth educators, consumers, hospital administrators, and liability insurance providers, and created a consensus statement that included, among other things, a mandate to create a more integrated system in the U.S. Community-based midwives have embraced this concept, and most have policies to guide their part in the process. However, a decade later, this mandate is still not operationalized in many hospitals. In this session, the presenters will present readily-available, free, user-friendly tools and guidelines for creating seamless transfer policies and procedures, as well as ways to help hospitals incorporate these processes into their systems. Clinical exemplars will be included from some hospitals who are using them, and from some who are not.

Faculty

Mairi Breen Rothman, CNM, DM, FACNM
Dr. Máiri Breen Rothman completed her C NM at Frontier School of Midwifery & Family Nursing, MSN from Case Western, and Doctorate in Midwifery from the Midwifery Institute at Jefferson. A fierce advocate for women’s sovereignty, Máiri has been a midwife since 1996, practicing in home, birth center, clinic, and hospital settings. She currently directs a home birth service, M.A.M.A.S., Inc. She also co-directs Home Birth Academy, offering online and in person workshops on community-based birth and seamless transfer. Máiri has taught both clinically and in the classroom at multiple universities. She earned the first-ever ACNM Policy Award. Máiri has presented on seamless transfer and other topics at ACNM, MANA, ICM, and the International Conference on Normal Labour and Birth Research. She was a contributing author to the ACNM Home Birth and Birth Center Manual. In her spare time, Máiri leads a choir, and enjoys changing laws and traveling.

Susanrachel Condon, CM, DM, FACNM
Dr. Susanrachel Condon earned a Midwifery Certificate from SUNY Downstate, and a Doctorate in Midwifery from Jefferson University. Since 2004, Susanrachel has co-owned a practice in New York, providing primary care and home-based perinatal services. She has also worked in hospital and clinic settings. Susanrachel has been a preceptor for midwifery and nursing students and is currently full time faculty at SUNY Downstate. As a member of the Home Birth Integration Initiative of New York she participated in the development of evidence-based practice guidelines and has contributed to the ACNM Home Birth & Birth Center Manual. Her doctoral research centered on obstetric violence, respectful care, and midwives as role models for collaborative decision-making. She has presented widely on the topic of respectful care. Susanrachel has been published in JMWH and Midwifery, and is a peer reviewer for JMWH and The Journal of Complementary & Alternative Medicine.
